Fill out this form for all "in-house" events that meet the following parameters
- Goes past 9:00 pm
- Involves alchohol in any way
- May Exceed 100 people total
NOTE: Once submitted, the form will be sent directly to the Corvallis Fire Dept. Greek Life will not receive notification of your event.
